How do I book a Plum Tree memory assessment?

1. Connect with your primary healthcare provider (family physician or nurse practitioner) to obtain a referral.

If you are uncertain about how to make this request, we've prepared a letter that you can share with your health care provider to assist you in effectively communicating your interest in being seen at our clinic.

2. Once we receive the completed referral forms, our team will contact you to book your initial appointment.

We are able to prioritize appointments based on need.

3. After your appointment is successfully booked, you will receive a comprehensive welcome package.

This package contains valuable information on how to prepare for your visit with us. Please print and complete the forms included in this package and bring them with to your first appointment.

Frequently asked questions

Before your first visit with us:

  • As we age, many of us notice changes in our memory, language, thinking, and personality. However, if these changes begin to disrupt your daily routines, it’s important to consider a memory assessment to ensure your brain is functioning at its healthiest.

    The following symptoms may indicate memory loss:

    • Difficulty performing daily activities, including changes in work performance;

    • Increased forgetfulness or trouble with short-term memory;

    • Needing frequent reminders to remember important information (e.g., frequently-used passwords, bank card PIN, etc.);

    • Repeatedly asking the same questions or forgetting recent conversations;

    • Forgetting appointments, special occasions, or medication schedules;

    • Struggling to find the right words to express yourself or experiencing lapses mid-sentence;

    • Regularly misplacing items like keys or wallets;

    • Challenges in managing finances, such as writing cheques or banking;

    • Becoming disoriented or lost in familiar surroundings; and

    • Changes in personality, including feelings of sadness, irritability, or diminished interest.

    If you or someone you know is experiencing some or many of these symptoms, contact your primary health care provider as soon as possible and request an evaluation. Early detection of cognitive issues generally leads to improved outcomes and a better understanding of your specific condition for suitable treatment options.

  • Plum Tree Memory Care’s assessments are available to anyone living in Ontario once we have received a referral from their primary health care provider. There are no age restrictions or minimums for an assessment. We are also happy to provide a second opinion if you have already received a diagnosis.

  • Getting a diagnosis and finding the right care can be challenging. Our team of friendly and highly-skilled health care professionals is ready to help. Plum Tree Memory Care assessments provide a comprehensive picture of your brain’s cognitive function. This provides our health care team with the necessary information to determine a diagnosis and a care plan that best manages your memory concerns.

    Our memory assessments rely on the latest breakthroughs in memory care research to guide you in living well and safely with memory loss. We will work with you and your family to create a uniquely-tailored care plan focused on your goals for living with memory loss confidently and with dignity.

  • The decision to seek medical help is up to the individual. By providing them with information, support, and understanding, you can increase their awareness and willingness to consider the benefits of our memory care services.

  • You will need a referral from your primary health care provider (family doctor or nurse practitioner) to ensure we work together to provide seamless care to you and your family.

  • To ensure we can do a thorough assessment, all initial memory assessments are done in-person at our office, located in downtown Kitchener. After the initial assessments, virtual assessments can be considered if needed.

  • Plum Tree memory assessments are based on the award-winning MINT Memory Clinic model developed by Dr. Linda Lee, founder and lead physician of both Plum Tree Memory Care and the MINT Memory Clinic model. MINT Memory Clinics have been implemented in over 100 sites across Canada. During your visit:

  • Memory assessments take place over two visits to ensure your experience is comprehensive. At each visit, you will meet with our highly skilled team of health care professionals.

    If requested by your primary health care provider, at the end of the second visit you will have the opportunity to meet with a Plum Tree Memory Care physician, who will provide a detailed OHIP-covered assessment, including discussion of diagnosis and recommended plan of management. Our physician services are backed by additional specialist expertise when needed.

    Recommendations may include further medical testing, medication optimization, and community supports and services that can enable you and your family to live well at home.

  • Your first visit will last approximately 1.5 to 2 hours.

    Your second visit will last approximately 1 to 1.5 hours.

  • Your first visit will include the following:

    • Thorough review of individual and family concerns;

    • Assessment of function and safety, including driving safety;

    • Comprehensive cognitive testing; and

    • Introductory information on protecting brain health.

  • Your second visit will include the following:

    • Medication review;

    • Physical assessment; and

    • Personalized brain health information, including discussion of lifestyle recommendations that can help you protect your memory.

    At the end of your second visit, the Plum Tree Memory Care team will provide you with a personalized care plan and suggested next steps on your memory care journey.

  • Yes! We encourage you to bring a trusted family member or friend with you to help provide additional details on any memory concerns you have and support you during your assessment. Care partners often provide valuable insights and specific examples of memory concerns, which can be helpful for a comprehensive understanding of your memory-related challenges.

    If you intend on attending your appointment alone, please contact our office ahead of time so we can accommodate your visit.

  • Yes, the cost of the assessment includes both the first and second visit.

    If you wish, at the end of your second visit, you may be offered a follow-up appointment for reassessment. These reassessments are offered at additional cost.

  • The memory assessment is comprehensive and includes services provided by our nurses, occupational therapists, pharmacists, and social workers. These are not covered by OHIP and there will be a cost to you for these services. Some private insurance plans may provide partial or full coverage depending on your plan. You can obtain a detailed receipt for the health services rendered by each health care professional upon request.

    The physician services provided through Plum Tree Memory Care are paid for by OHIP for patients residing in Ontario with a valid OHIP health card.

After your visit:

  • Results and recommendations from your memory assessment will be sent at the end of your second visit to your primary health care provider (family doctor or nurse practitioner) who is willing to receive and act on the information provided.

    Alternatively, your primary health care provider can request that you receive a detailed assessment by a Plum Tree Memory Care physician on staff who is trained in memory care and will interpret results of various assessments and information and send recommendations to your primary health care provider (family doctor or nurse practitioner). There is no requirement that the assessments and information reviewed by the Plum Tree Memory Care physician is obtained through a Plum Tree memory assessment.

  • For optimal brain health, we recommend booking a memory assessment every 6–12 months. If you are experiencing a decline in your cognitive brain health, we recommend booking a memory assessment every 6 months to carefully monitor changes and address them immediately for improved health outcomes.
