Navigating memory challenges is easier when you’re not alone. Our better together coaching program offers personalized one-on-one coaching sessions with health care professionals to help individuals and families enhance their interactions with those they care about living with memory challenges.

Personalized coaching

Our program offers one-on-one coaching sessions with licensed health care professionals with expertise in dementia care, helping individuals and families to create meaningful, harmonious relationships with those they care about living with memory challenges.

Our Better Together Coaching program includes three (3) sessions with a Plum Tree health care professional for personalized support and guidance tailored to each individual's or family's needs. During these sessions, we can help identify strategies to enhance social interactions, improve communication, and address specific memory-related challenges you are facing to improve confidence and resilience. Our goal is to help you to live better when someone you care about has memory challenges.

Our program

Our coaching program consists of three (3) sessions facilitated by our expert health care professionals. Depending on your needs, sessions can focus on communication strategies, social interaction techniques, stress management, and coping skills tailored to the individual. You will be asked to complete an intake form prior to attending your first coaching session to help our experts customize meaningful solutions for you.
