Our Seniors’ Clinic is a full-service primary care clinic for Ontario residents age 65+, conveniently offered at our Kitchener clinic. The clinic is nurse practitioner-led and is supported by other health care professionals. The Plum Tree Seniors’ Clinic specializes in providing care to older adults and is committed to providing top quality, comprehensive primary care.

About the Clinic

The Plum Tree Seniors’ Clinic is a nurse-practitioner-led full service primary care clinic for people age 65+.

If you are age 65+ and do not have another primary care provider, you can enrol in the Seniors’ Clinic.

The Seniors’ Clinic and the health care professionals supporting the team will be covered by OHIP. Certain services provided by the clinic are not covered by OHIP (see below).

Some of the services provided by the Clinic are not covered by OHIP. These include:
Forms/certificates
Liquid nitrogen treatment or skin lesion removal
Private consultations and visits requested by non-OHIP card holders or third parties
Transfer of records
Letters for therapy or medical devices
Missed appointment without cancellation (24 hours)
NSF cheque

All health records are stored securely on our computer system with the Centre for Family Medicine. They are held in strict confidence according to the Personal Health Information Privacy Act. If you provide us with your email address, we will send you information via email and ask that you read these emails when received.

The Clinic is nurse practitioner-led and supported by other health care professionals, including family physicians, nurses, and pharmacists.
Plum Tree sometimes provides mentorship and learning opportunities for health professional learners. You may occasionally be seen by learners who will always be supervised by qualified health professionals.

The office is co-located at 16 Andrew Street, Kitchener, N2H 5R2, with Plum Tree Memory Care and the Centre for Family Medicine Regional Memory Clinic.

Yes, there is parking at our site. Parking is $3 and is payable at the gate when exiting. You can use loonies or toonies or tap with debit/credit.

Most appointments are booked for 30 minutes.

Unfortunately, walk-in appointments cannot be accommodated.

For urgent issues, we have same day and after-hours coverage available every day except for some holidays. Please do not use walk-in clinics.
To access the after-hours clinic, please call (226) 355-3201, which will direct you to which location is on call and the phone number to call. When the after-hours clinic is closed, please call Tele-Health Advisory Service (THAS) at 1-866-553-7205.
In case of emergency, please call 911 or go directly to the nearest emergency department.

An appointment may be required for prescription renewals, especially for new medications or after adjustments. Prescription renewal requests may take up to three business days. Your pharmacy may have refills on file. Please check with your pharmacist at least three business days before running out of your medications.

Plum Tree has a zero-tolerance policy for offensive and abusive language, as well as physical violence. Serious or recurrent unacceptable behaviours are grounds for dismissal from our clinic.
